Application form and Summary of Benefits may be requested through email Maxicare healthcards and booklets are now available. If interested please print and submit application form, together with your check payment payable to the Philippine Dental Association. Premium amount of Php.11,000.00 for Principal members under MAXICARE. Coverage period is for January 31, 2011 to January 30, 2012 for Pph.120,000.00 maximum benefit limit per dreaded disease limit per year. This program is open to all PDA members and their Clinic Staff as Principal Enrollees. Chapter Seminar Orientation for Maxicare Benefit Coverage for this term may also be arranged by request.
